The following QCUCAT PASSERS can now submit their Enrollment Requirements at QCU SAN BARTOLOME CAMPUS in order to secure their slots. Please STRICTLY FOLLOW YOUR SCHEDULED APPOINTMENT, LATECOMERS AND WALK-INS will not be accommodated. Please refer to the back of the QCU Admission Application form for the list of Enrollment Requirements.

FRESHMAN

SHS GraduateS, High School GraduateS, ALS GraduateS, Or Tech-Voc Graduates Of 6-Month Or Short Courses

1. Fully Accomplished QCU Admission Application Form (printed back to back on a single sheet of paper) with 2×2 colored ID picture with white background, name tag, and applicant’s signature affixed on the space below the form.

2. SHS GRADUATE – Original SF9 (Certified True Copy of Grade 11 & Original Grade 12)
HIGH SCHOOL OLD CURRICULUM – Form 138- High School Report Card
ALS GRADUATE – Alternative Learning System Certificate of Rating

3. Original PSA Birth Certificate with PSA Official Receipt

4. Original Certificate of Good Moral Character

5. Original Barangay Certificate of Residency (recent)

6. Two(2) 2×2 pictures with name tag (white background)

7. Photocopy of Senior High School / High School / ALS Diploma (present original)

8. Medical Clearance from the QCU Medical and Dental Services (refer to the instructions below)

9. Long brown envelope with a plastic envelope

10. Original copy of Certificate of Employment (if currently employed)

11. Original Marriage Certificate (for married female students)


TRANSFEREE

An applicant who has taken college programs or units and/or has taken 1 to 2 of years vocational or any special training in recognized colleges or universities.

TRANSFEREE-2ND DEGREE COURSE TAKER

if graduated in any Degree Course

1. Fully Accomplished QCU Admission Application Form (printed back to back on a single sheet of paper) with 2×2 colored ID picture with white background, name tag, and applicant’s signature affixed on the space below the form.

2. Transcript of Records (for evaluation purposes)

3. Original PSA Birth Certificate with PSA Official Receipt

4. Original Certificate of Transfer Credentials

5. Original Certificate of Good Moral Character

6. Original Barangay Certificate of Residency (recent)

7. Two(2) 2×2 pictures with name tag (white background)

8. Medical Clearance from the QCU Medical and Dental Services (refer to the instructions below)

9. Long brown envelope with a plastic envelope

10. Original Certificate of Employment (if currently employed)

11. Original Marriage Certificate (for married female students)

12. For Accreditation of Subjects: Subject Course Description and NSTP 2 Serial Number


You should complete your medical requirements before the submission of documents. INCOMPLETE MEDICAL REQUIREMENTS WILL NOT BE ACCEPTED. Medical requirements must include the following:

  • Chest X-ray
  • CBC
  • Urinalysis
  • Medical Certificate.

At the discretion of the physician who examined you, you may be required to undergo additional tests.

Steps to Secure medical clearance from medical and dental services

1. Email medicalservices@qcu.edu.ph for the medical referral which you may present as a request to the hospital/clinic of your choice.

2. Submit PERSONALLY the original copy of the following at the QCU Medical Clinic in San Bartolome Campus:

  • Chest X-ray result
  • Complete Blood Count Result
  • Urinalysis
  • Medical Certificate.

3. Provide one (1) photocopy of each result.
